In-home services are designed to help older adults and individuals of all ages with disabilities live independently in their homes and communities, as opposed to living in and receiving care in a facility such as a nursing home. - Personal Care - Advanced Personal Care (any care required for an altered body function) - Household Chores - Respite Care (to provide relief for primary caregivers) - Nursing Visits

HCY Cooperative Home Care & The Healthy Children and Youth Program Cooperative provides home care services for children under the age of 21 through the Healthy Children and Youth (HCY) Program, funded by the Missouri Bureau of Special Health Care Needs (BSHCN). Available services include basic and advanced personal care services and authorized nurse visits. The purpose of the authorized nurse visit is to provide increased supervision to the personal care aide, intensified health care assessment of the recipient and in some cases, certain nursing services that would not meet the requirements for reimbursement under either the Medicare or Medicaid Home Health Programs. The authorized nurse visit is used for advanced personal care plan development, on-the-job training for the advanced personal care aide and monthly evaluation of advanced personal care recipients.
