Our Service
Private services span from mini-visits and hourly care to 12-hour and 24-hour shifts and live-in care. Private services include skilled nursing, therapies, personal care, household chores, companion care, errands and appointments. Each service we provide is exceptional because our approach is specialized according to an individual’s needs. Our case management model is comprehensive, beginning with our free in-home assessment (link here), which provides us with a complete understanding of your needs and expectations. The in-home assessment will provide you with information about potential funding sources for home care and about Cooperative Home Care. After the assessment, we provide a complimentary, individualized Care Plan. Services begin when you need them. Even if your need is last-minute or an emergency, we will be there.

12 and 24-Hour Care Specialist
Our 12 and 24-hour care focuses on recruiting, training and staffing procedures that reflect the unique requirements for the individual needing extensive care up to 24 hours a day. Live-in care is an affordable, flexible alternative to long-term care or assisted living. Our highly qualified Private Services Professionals are available to fill shifts quickly. Discounted shift rates are available and offer significant savings from our regular hourly rates.

Caregivers to Trust
Each caregiver is thoroughly screened and carefully selected for their traits of dedication, warmth and care. An experienced case manager will perform an assessment in your loved one’s home before we begin care and will continue to see all aspects of care including insurance issues, additional nursing needs as well as physical, speech and occupational therapies.
Your case manager talks to you regularly about your loved one and ensures the quality and consistency of care.
